Your contact information, including your name, address, phone number, and email address.
A statement under penalty of perjury affirming that you believe in good faith that the material was removed by mistake or misidentification.
A statement consenting to the jurisdiction of the federal district court where your address is located (or, if you live outside the United States, the jurisdiction of any court where the Site may be found) and agreeing to accept legal notifications from the person who filed the DMCA notice or their agent.
